Alexandra Clinic was forced to close for a day after staff in the casualty unit were held up by gunmen on Monday morning.
The incident follows a botched hijacking outside the clinic last month during which a masters student was shot dead and has raised more security concerns at the facility despite an agreement from the police to guard the area.
Clinic head Labane Maluleke said the latest incident had left his staff shocked and traumatised.
He said that around 7am on Monday, two men arrived at the clinic with their friend, who they said had been injured in a mob attack.

"While nurses attended to his friend, one man came back from outside and pulled out a gun and started harassing staff and threatening to shoot.
"They then grabbed their friend and walked out," said Maluleke.
