The tragic passing of Professor Bongani Mayosi has been a reminder of our need to build psychologically safe environments where all the role-players, regardless of their seniority, are not fearful of being humiliated or disciplined for sharing their concerns, ideas or mistakes.

Although not confined to this industry, it is particularly so in healthcare, where the climate - and the way in which the authority gradient is addressed - needs to encourage healthcare professionals to raise their hands where errors have occurred or where they believe that patient safety is compromised.

Equally importantly the management of these organisations need to acknowledge their own fallibility in not being able to solve every difficult dilemma, often including ethical aspects, without the assistance of their subordinates.

Balancing accountability with psychological safety is vital.

It is in embarking on this journey of respectful collaboration that we have the potential to effect the greatest shift in organisational culture, particularly where transformation is a vital element on the agenda.
